Non-Invasive Cardiac Imaging

Transthoracic echocardiography (TTE) with two-dimensional, M-mode, Doppler, and color flow mapping capabilities enables comprehensive assessment of left ventricular systolic function including volumetric quantification of ejection fraction via the modified Simpson biplane method, evaluation of diastolic dysfunction graded according to the 2016 ASE/EACVI consensus algorithm, characterization of valvular morphology and hemodynamic severity using the continuity equation and proximal isovelocity surface area (PISA) methodology, and identification of intracardiac thrombus, pericardial effusion, and congenital structural abnormalities.

Ambulatory electrocardiographic monitoring via 24- to 48-hour Holter recording or extended 30-day event monitoring provides continuous rhythm surveillance for the detection of paroxysmal atrial fibrillation, supraventricular and ventricular arrhythmias including the quantification of premature ventricular complex (PVC) burden, intermittent pre-excitation syndromes, and conduction system abnormalities including sinoatrial node dysfunction and atrioventricular block of varying degrees.

Commonly Managed Conditions

  • Hypertensive cardiovascular disease: Pharmacological management utilizing ACE inhibitors, angiotensin II receptor blockers (ARBs), dihydropyridine calcium channel antagonists, and thiazide-type diuretics, titrated to target blood pressure thresholds consistent with the 2017 ACC/AHA Hypertension Guidelines.
  • Coronary artery disease (CAD): Risk stratification using the HEART score and CHA2DS2-VASc algorithm, antianginal therapy optimization, and coordination of percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) referral in accordance with appropriate use criteria.
  • Heart failure with reduced ejection fraction (HFrEF): Guideline-directed medical therapy (GDMT) comprising angiotensin receptor-neprilysin inhibitors (ARNIs), evidence-based beta-blockers, mineralocorticoid receptor antagonists (MRAs), and sodium-glucose cotransporter-2 (SGLT2) inhibitors, demonstrating mortality benefit across multiple landmark trials including PARADIGM-HF, MERIT-HF, RALES, and DAPA-HF.
  • Atrial fibrillation (AF): Rate versus rhythm control strategy selection, direct oral anticoagulant (DOAC) initiation for stroke prophylaxis based on CHA2DS2-VASc risk score, and evaluation for catheter ablation candidacy per current Heart Rhythm Society consensus statements.
  • Dyslipidemia: High-intensity statin therapy, ezetimibe co-administration, and PCSK9 inhibitor initiation for patients with familial hypercholesterolemia or residual atherosclerotic cardiovascular disease (ASCVD) risk despite maximally tolerated statin dosing.
